Billing & Insurance
We participate with most local and many national insurance plans. However it is your responsibility to understand whether your insurance has limits on the doctors you can see, or the services you can receive.
If you provide complete and accurate information about your insurance, we will submit claims to your insurance carrier and receive payments for services. Depending on your insurance coverage, you may be responsible for co-payments, co-insurance, or other deductible amounts.
Important note: Please remember to bring your insurance card and a photo ID with you to each appointment. If you don’t bring your insurance card, we will request payment in full for your visit. If you forget your photo ID, you may have to reschedule your appointment. Federal laws against identity theft now require us to verify our patients’ identities.
Financial Policy
Insurance Co-pays
It is our responsibility to your insurance company to collect co-pays at the time of service. So please be prepared to make any co-pays required by your insurance plan at each visit to avoid possibly having to reschedule.
Payment methods
We accept cash, check, CareCredit, Visa, MasterCard and debit cards.
Delinquent accounts
If your account becomes delinquent, please contact our billing department to set up a payment plan. Delinquent accounts may be assigned to a collection agency. All collection costs will be added to your account
Returned checks
Returned checks will be charged a $25 service fee.
