What are your office hours?

Our office is open from 8 a.m. to 5 p.m. Monday through Thursday and from 8 a.m. to 3 p.m. on Fridays.

What if am late for my appointment?

If you arrive more than 15 minutes late for your appointment, you may be asked to reschedule.

What should I do if I have a medical question during office hours?

If you have a question about your medical care during normal business hours, please call our office at (336) 333-6306 and select the option connecting you to the clinical staff triage voice mail. A member of our clinical staff will retrieve your message, review your questions with our medical staff, and respond to your call. Calls are returned to patients throughout the day.

How can I get a prescription refill?

When you need a prescription refilled, please call your pharmacy. For drugs requiring a written prescription, please call us at (336) 333-6306, select the option connecting you to the clinical staff triage voice mail, and leave a message.

How can I get copies of my records?

Requests for medical records can be made at (336) 333-6306 option 6. If you have questions, please contact our Medical Records Department at (336) 333-6306 Ext 1205.

What is the procedure for getting a form completed?

If you need a form completed, please fill out the patient portion of the form, You can fax your form to us at our main fax number, (336) 333-6309, or you can mail or hand-deliver the form to our office. The fee for form completion is $30. Please note that we require 10 business days for completion of disability forms.

What should I do if I have an after-hours emergency?

If you need medical attention when the office is closed, please call the main number at (336) 333-6306 and you will be connected to our answering service. If you have a life-threatening emergency, dial 911 or go to the nearest emergency department.
